APPOINTMENTS
We have made requesting an appointment a simple process. To request an appointment via our website, simply click on the link above and it will give you step by step instructions. You can also phone any time during business hours and our staff would be happy to assist you.

PAYMENT POLICIES
We ask that all co-payments and co-insurance be paid on the date of service and we gladly accept checks, cash or credit cards. Our office now also accepts American Express. Please feel free to call with any questions.

INSURANCE & BILLING
We accept and bill most insurance plans. Please contact our office to verify acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of your benefits.
